Under provisions of the Hospital Report Card Act, consumers are permitted to receive, upon request, information about nurse staff schedules, assignment rosters, methods for determining and adjusting staffing levels, and staff training information. To make such a request, persons must complete the Request for Information – Illinois Report Card Act form.
Once the signed form has been submitted, La Rabida Children’s Hospital will provide the appropriate information either verbally or in a format that may be viewed by the requestor. No documentation will be provided for a requestor’s retention purposes.
Once the information is provided, the requestor will be asked to sign a form acknowledging receipt of information provided under the Illinois Hospital Report Card Act.
Under no circumstance shall any information containing data specific to any individually identifiable patient, employee, or licensed professional be disclosed to the public.
