La Rabida Care Coordination
We assess the needs of children with complex medical issues and work closely with parents, caregivers, primary care physicians and specialists to develop a plan of care that is unique to each child we serve.
We work with providers and health plans to make sure children with special health care challenges can carry out their treatment plans and find the people, services and equipment they need. Our comprehensive approach to supporting the overall health of our patients is effective for the children, comforting for their families and caregivers and highly valued by health care providers, schools and other stakeholders in the communities we serve.
Our holistic and family-centered plans ensure that health care services are accessible, coordinated and timely. Not only do we focus on the medical aspects of care, but we also work with families and caregivers to identify and eliminate barriers to assessment and treatment including access to food, housing and transportation.
Our methods take a community-based approach and include conducting home visits, accompanying children to medical appointments, supporting parents/caregivers by attending Individualized Education Program meetings and providing transportation assistance for medical appointments.
La Rabida’s care coordinators are highly experienced and available to assist families in a variety of pediatric areas including neonatal intensive care services, pediatric intensive care unit, pediatric and adolescent behavioral health and early intervention. They are also experts in connecting families and caregivers with services such as Department of Children and Family Services, Division of Specialized Care for Children and foster care.
In addition to coordinating a path for families to access services directly, our care coordinators provide information about behavioral health, mentoring programs, nutrition, summer camps and support groups.
